New Passport Fee Schedule Starting February 1, 2008

Passports

The Consular Section issues new and replacement U.S. passports and provides additional passport pages as necessary. See Special Requirements for Children below.

Additional information on Passport Services is available from the Bureau of Consular Affairs. Please note that we are no longer able to amend Passports.

To renew a valid U.S. Passport, the following are required:

-Your most recent U.S. Passport, issued since your 16th birthday and within the last 15 years; Passport application form DSP-82 (the blue form, available at the Consular Section or online by clicking here);

-Two recent identical photos, 2x2 inches, taken on a white/light color background. (The head size should be about one inch in length.);

-Your Social Security number, if issued; and
-A fee of US$75 in cash.

To replace a lost/stolen passport, the following are required:

Proof of U.S. Citizenship (e.g. an original or certified copy of U.S. birth certificate, or a U.S. naturalization certificate, or a U.S. certificate of citizenship, or Consular Report of Birth Abroad); Proof of identity (e.g. valid photo identification issued by a State or the U.S. Government); Passport application form DSP-11 (the peach-colored form, available at the Consular Section or online by clicking here).

Police report and an affidavit regarding the circumstances of the loss/theft (blank affidavit DS-64 forms are available at the Consular Section or online by clicking here);

Two recent identical photos, 2x2 inches, taken on white/light color background. (The head size should be about one inch in length.); Your Social Security number, if issued; Fee of US$100 in cash (US$85 for children under 16).

To apply for your first U.S. Passport, the following are required:

Proof of U.S. citizenship (e.g. an original or certified copy of a U.S. birth certificate, or a U.S. naturalization certificate, or a U.S. certificate of citizenship, or Consular Report of birth abroad); proof of identity (e.g. valid photo identification issued by a State or the U.S. Government); Passport application form DSP-11 (the peach-colored form, available at the Consular Section or online by clicking here);

Two recent identical photos, 2x2 inches, taken on a white/light color background. (The head size should be about one inch in length.); Your Social Security number, if issued; Fee of US$100 in cash (US$85 for children under 16).

Special Passport Application Requirements for Children under the age of 16:

Passport application form DSP-11(the peach-colored form, available at the Consular Section or by clicking here;

Birth certificate with parent's names; Children under the age of sixteen should appear in person with both parents. If only one parent executes the application, that parent must establish sole authority to execute the application or the other parent's notarized written consent to the application.  A parent who will be absent must fill out in advance form DS-3053 (the yellow form, available at the Consular Section or online by clicking here);  Fee of US$85 in cash (US$60 for renewal of a valid passport for a minor).

To have additional visa pages inserted in your valid U.S. Passport, the following are required:

To apply for additional visa pages in a US Passport, please complete and sign form DS-4085 (the green form, available at the Consular Section or online by clicking here.)

Visa pages can be added to a valid, unexpired US passport by bringing the passport to the American Citizen Services (ACS) unit during walk-in business hours, Monday through Thursday, 1:00pm to 4:00pm and Friday, 8:00 am to 11:30 am and 1:00pm to 4:00pm, excluding Lao and American holidays.

The process usually takes less than one hour and is free of charge. If a representative will bring your passports and/or your child’s passport to the Embassy, please make sure the representative brings signed authorization letters from both of you, and valid photo I.D., and the signed application.

Note: When applying for visa pages for a child who is under 16, at both parents must sign the application; or the absent parent must fill out in advance form DS-3053 (the yellow form, available at the Consular Section or online by clicking here). It is not required that the child appear in person. However, a Consular officer may request further documentation if there are concerns about the identity of the child or the relationship between the parents.
